About this Opportunity
Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. Form Health provides high-quality expert care and leverages technology to enhance the patient experience. All Form Health patients work closely with their care team, which includes board certified physicians, advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. We hold ourselves to the highest standards of clinical care, and to treating every individual with empathy and respect.
Founded in 2019, Form Health is a venture-backed innovative startup with an experienced clinical and leadership team. Our mission is to empower patients and be leaders in Obesity Medicine driving impact at a national scale. We are deeply invested in our core value to put patients first, and also deeply committed to creating a culture where every employee is valued and we learn and improve together.
Great care depends on great tools. Behind every patient visit is a care team of physicians, advanced practice professionals, and Registered Dietitians who chart, coordinate, schedule, and manage longitudinal treatment across tens of thousands of patients. The Care Team Experience (CTX) team owns the systems that make that work possible: clinical charting, scheduling, task management, care coordination, EHR integration, operational tooling, and the workflow automation that lets a clinical team deliver safe, effective care at scale.
We're hiring a Senior Software Engineer to join CTX and take ownership of complex, safety-sensitive systems from design through production. This is a hands-on role for someone who wants to build backend services and workflows that clinicians rely on every hour of every day, in a regulated environment where reliability and correctness genuinely matter. You'll own meaningful features end to end, make sound architectural calls, and help set the technical bar for the team through your craft and your influence on the engineers around you.
We're an AI-native engineering org with a continuous delivery culture. We ship in small batches, lean heavily on agentic development workflows (Claude Code is a core part of how we work), and measure ourselves on both throughput and stability. We're looking for someone who already works this way, or is excited to.
This role reports into the Engineering Manager for the Care Team Experience team and partners closely with product, clinical, and operational stakeholders. This is a fully remote position and is open to candidates located anywhere in the United States.
Design, build, and ship backend services, workflows, and integrations that power clinical charting, scheduling, task management, and care coordination
Own features end to end: from shaping the approach with product and clinical partners, through implementation, testing, rollout, and operation in production
Work primarily in a mature Ruby on Rails monolith, alongside supporting services and internal tooling
Build and maintain integrations with clinical and operational systems, including our EHR
Practice small-batch, continuous delivery: small PRs, short-lived branches, feature flags, and tight feedback loops that let us ship safely and frequently
Use agentic, AI-native development tools (e.g., Claude Code) as a core part of how you design, build, test, and review, without compromising engineering rigor
Write well-tested, observable, maintainable code, and instrument your work so we can understand how it behaves in production (we run on Datadog)
Raise the technical bar for the team through thoughtful code review, pairing, and mentorship of other engineers
Ensure the systems you build operate safely within HIPAA and broader healthcare regulatory requirements
Identify technical risks early and help drive mitigation before they impact care delivery
You are a senior individual contributor who combines strong technical depth with a bias toward shipping. You've owned complex, production-grade systems and care about doing it well in environments where correctness matters.
5+ years building and operating production software, with meaningful backend and workflow-heavy experience
Strong proficiency in a modern backend stack; experience with Ruby on Rails is a strong plus, and comfort ramping quickly in an established codebase is essential
A track record of owning features end to end and making pragmatic architectural decisions
Strong delivery fundamentals: small-batch and continuous delivery, feature flags, automated testing, and operational excellence
Comfort with cloud infrastructure and observability (we run on AWS and Datadog)
Experience integrating with third-party systems and APIs
Hands-on fluency with, or genuine enthusiasm for, agentic AI-native development tools (e.g., Claude Code) as part of your everyday workflow
Clear communication and effective collaboration with product, clinical, and non-technical partners
Experience in healthcare, clinical systems, workflow tooling, AI-enabled products, or other regulated domains is a strong plus, but not required
Your work will directly shape how Form Health delivers care at scale. You'll help build systems that support:
Reliable clinical and operational workflows
Secure, HIPAA-compliant patient data handling and care delivery
Scalable infrastructure and integrations
Intelligent, safe workflow automation with human oversight
Safe, incremental experimentation and continuous improvement
Competitive salary and equity in a high growth start-up
Comprehensive health benefits (medical, dental, vision)
401k
Flexible work schedules and paid time off
Paid parental leave
Form Health’s commitment to building a diverse, equitable, and inclusive work environment:
Form Health is committed to creating a culture and environment that celebrates diversity and inclusion, while fostering safety and belonging. This extends from our remote patient care to our corporate offices and everywhere in between. We are looking for team members who want to help us further our Diversity, Equity, and Inclusion (DEI) efforts and who share our attitudes for creating an inclusive, safe, and positive work environment.
