FormKiosk is feedback collection for businesses with a counter: cafés, hotels, clinics, salons. You build a branded form, then put it on a QR code or run it on a tablet at the till. Someone taps a face, it thanks them, counts down and resets for the next person. The Android app draws the form natively, so it keeps collecting when the café wifi drops and sends the answers when the connection returns. A tablet pairs with a six-character code instead of a password, and what it holds is a grant scoped to one form, not a login: it cannot read the answers back or reach billing. Every number the business runs on is published at formkiosk.com/open, including the embarrassing ones.
